Personal Declaration Forms

As part of the license application, certain forms must be accurately filled out and submitted to the GSC. This includes:

- The Personal Declaration Form (PDF) Controlled Roles for individuals in controlled roles (i.e Directors) .

- The Personal Declaration Form (PDF) Non-Controlled Roles for individuals in non-controlled roles (i.e Shareholders).

- The Simplified Personal Declaration Form (SPDF), intended for individuals with non-controlling roles (i.e Investors)

All forms must include thorough, honest disclosures. Incomplete or inaccurate submissions will not be approved. The GSC may validate information, request additions, and interview applicants.

If a new individual takes on a key controlled role after licensing, business must submit a PDF to the GSC for that person. Ongoing compliance with this process is expected.

The documentation aims to establish integrity for the regulated cannabis industry on the Isle of Man. A correct form submission to the GSC is required for license application review.

Ultimate Parent Company Form (UPC)
